How do I insert an image by my user name?

When setting up your profile there should be a spot that says insert avatar. That is where you want to put it.

Yeah, I had to upload my image again for it to work. Was it due to the updates or something to do with the server?

the half pictures are because of the upgrade.. if you reload them they will be fine, there is a thread about this somewhere!
